S/N #70-2461

Mishap aircraft stationed with Det 3, 37th ARS. Aircraft was on Grand Forks TACAN 300 degree Radial, 30.5 DME.

While on a medevac mission for an injured missile maintenance technician, the aircraft crashed killing all four crew members (2 pilots, 2 med techs). Main rotor separation following mast bumping due to low rotor RPM. No definite cause identified.
